Darlington PCN Recognised For Quality Diabetes Care
Through the National Diabetes Audit, Darlington Primary Care Network (PCN) has been identified as one of the top two performing areas in the country for diabetes care among young adults with type 2 diabetes.
This recognition highlights the success and efforts of the integrated community care model.
Through this integrated approach all type 2 diabetes patients are managed within the community, with support from secondary care. This collaborative approach ensures continuity and quality of care.
